company, depending on symptoms or difficulties you may be having, along with benefit
provisions of your insurance plan. You may wish to contact your insurance carrier to
determine if such surgery is considered a benefit under the terms of your contract. Final
determination would need to be made after consultation with Dr. Ghafoori and submittal to
your insurance company for Pre-Authorization/Pre-Determination.
